Comprehensive Coaching Agreement

This form has two goals. One is to collect relevant information about you to help us better serve you. The other is a contract to make sure we all have clear expectations of our responsibilities and boundaries for the coaching relationship between Blue J Coaching LLC (Jacob Santhouse, EdD) and the client. Please read carefully, enter requested information, and provide your signature to confirm agreement to this contract.

1. Scope of Coaching Services

The Coach agrees to provide coaching services to the Client aimed at personal development in service of developing strengths and accomplishing goals. Coaching is a partnership between the Coach and Client in a thought-provoking and creative process that inspires the Client to maximize personal and professional potential. Coaching is not therapy, counseling, medical advice, legal advice, or financial advice. The Client understands that coaching is not a substitute for professional mental health care or medical care.


2. Confidentiality

All discussions between the Coach and Client are strictly confidential. No information will be shared with third parties without the Client's explicit written consent, except in the following circumstances:

  • If the Coach believes there is an imminent risk of harm to the Client or others.

  • As required by law (e.g., court order, subpoena).

  • For supervision or training purposes, where client identity will be anonymized.


3. Client Responsibilities

The Client agrees to:

  • Be open to the process of coaching and show up prepared to engage with the process to the best of their ability.


4. Coach Responsibilities

The Coach agrees to:

  • Provide a safe, supportive, and non-judgmental environment.

  • Listen actively and ask powerful questions.

  • Challenge the Client constructively and offer different perspectives.

  • Maintain professional boundaries and ethical standards.

  • Keep all client information confidential as outlined above.


5. Fees and Payment

The total fee for Pricing based on single session is $150 USD per session.

Payment Schedule: Full payment is due at time of booking (unless otherwise agreed upon by yourself and the Coach).

Payment Methods Accepted: Credit/debit cards via Stripe payment processing.

Late Payment Policy: 1% late fee per day will be applied beginning after 30 days.


6. Cancellation and Rescheduling Policy

Sessions canceled less than 24 hours before the session will result in a $50 late cancellation fee. The Coach will make reasonable efforts to reschedule sessions when sufficient notice is given.


7. Termination of Agreement

This Agreement may be terminated by either party at any time. In the event of termination by the Client, any previously booked sessions are non-refundable. In the event of termination by the Coach, any previously booked sessions will be refunded to the Client on a prorated basis.


8. Refund Policy

All sales are final once coaching services commence.


9. Disclaimer

The Client understands that Coaching is not a substitute for mental health counseling. The Client is solely responsible for creating and implementing their own physical, mental, and emotional well-being, decisions, choices, actions, and results arising out of the coaching relationship.


10. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of Florida/USA.


11. Entire Agreement

This Agreement constitutes the entire agreement between the Coach and the Client and supersedes all prior discussions, negotiations, and agreements, whether written or oral.


12. Privacy Policy

This Privacy Policy describes how Blue J Coaching LLC ("we," "us," or "our") collects, uses, and protects the personal information of our coaching clients and website visitors.


A. Information We Collect

We may collect the following types of personal information:

  • Contact Information: Name, email address, phone number, mailing address.

  • Demographic Information: Age, gender, occupation (if relevant to coaching services).

  • Coaching-Related Information: Information shared during coaching sessions, intake forms, assessments, and progress updates. This may include personal goals, challenges, experiences, and sensitive information relevant to your coaching journey.

  • Payment Information: Billing details, credit card information (processed securely via third-party payment processors). We do not store full credit card numbers on our servers.

  • Technical Data: IP address, browser type, operating system, referring URLs, and website usage data (collected via website analytics).


B. How We Collect Information

We collect information through various methods, including:

  • When you complete our intake forms or questionnaires.

  • During coaching sessions and communications (e.g., email, phone, video calls).

  • When you make payments for our services.

  • Through your use of our website (e.g., cookies, analytics tools).


C. How We Use Your Information

We use your personal information for the following purposes:

  • To provide and deliver our coaching services.

  • To schedule and manage appointments.

  • To process payments and send invoices.

  • To communicate with you regarding your coaching, updates, and relevant information.

  • To improve our services and tailor our coaching approach to your needs.

  • To comply with legal obligations.

  • For internal record-keeping and administrative purposes.


D. How We Store and Protect Your Information

We are committed to ensuring that your information is secure. We implement appropriate technical and organizational measures to protect your personal data from unauthorized access, disclosure, alteration, or destruction. These measures include Google Drive/Workspace.

We retain your personal information only for as long as necessary to fulfill the purposes for which it was collected, including for the purposes of satisfying any legal, accounting, or reporting requirements.


E. Sharing Your Information

We will not sell, distribute, or lease your personal information to third parties unless we have your explicit permission or are required by law to do so. We may share your information with trusted third-party service providers who assist us in operating our business (e.g., payment processors, scheduling software, video conferencing platforms). These third parties are contractually obligated to keep your information confidential and use it only for the purposes for which we disclose it to them.
